  1. Utilize myGDX to order collection packs for your office. Before providing to a patient, use the unique activation number assigned to each pack to complete the following:
  2. Login to myGDX and choose Order and Activate Tests
  3. Enter the activation number from the collection pack and select the specific tests and add-ons your patient needs
  4. Add billing, diagnosis, and patient info then Submit
  5. Ensure the label card is returned to the pack it was shipped in and provide to your patient for specimen collection

Healthcare providers begin activation by providing specific test orders and patients complete it with their signature and collection details.
